Advise the receptionist if you have had any previous breast surgeries or procedures. A physician's order for a mammogram is required.
On the day of your procedure:
Please check in at the Admitting Department. Bring your physician order, a photo ID and $50 (cash, check or credit card). You'll also fill out a brief information card at this time. Note: Your insurance will not be billed for this mammography screening.This is a screening only. Your results will be mailed to your physician. Special price offer must be paid in full at the time of service and cannot be used in conjunction with any insurance, Medicare or Medicaid. Offer applies toward a screening mammogram and is not valid for patients with breast implants or pre-existing conditions. Offer valid weekdays in October.
